Request for Proposals: Analysis of AMI and PN Readmission Model Data ProposalThe Colorado Foundation for Medical Care (CFMC) has received funding from Centers of Medicare & Medicaid Services (CMS) for a special project: “Medicare Quality Measurement Support Project: Mortality Implementation and Measures Development and Monitoring.” Obligations to this project include the analysis and bootstrapping of data for the project’s Acute Myocardial Infarction (AMI) and Pneumonia (PN) Readmission models. To best accomplish this, CFMC is requesting proposals from organizations with relevant, verifiable experience using CMS claims and enrollment data. For the purposes of proposing on this Special Project, the proposal must be prepared in two parts: (A) Technical Proposal and (B) Business Proposal.
REQUEST FOR PROPOSALS: Support for Implementation of the Dry Run for HF Readmission Measure ProposalThe Colorado Foundation for Medical Care (CFMC) has received funding from Centers of Medicare & Medicaid Services (CMS) for a Special Project: “Implementation of the Dry Run for the CMS 30-Day Heart Failure Readmission Measure.” Obligations to this project include the preparation of educational and informative materials related to, and participation in national presentations on, the CMS 30-day HF readmission measure for hospitals and other key stakeholders for the duration of this project. In addition, the project obligations include support to hospitals, QIOs and the public to respond to questions of a clinical and technical nature about the measure methodology and modeling, and performance of additional ad hoc analyses as requested by CMS. To best accomplish this, CFMC is requesting proposals from organizations with relevant, verifiable experience in the development of clinical measures and specific expertise related to the 30-day HF readmission measure development and methodology. For the purposes of proposing on this Special Project, the proposal must be prepared in two parts: (A) Technical Proposal and (B) Business Proposal. For each proposal, the Offeror shall submit all components of the proposal electronically via email, as directed below:

For the purposes of proposing on this Special Project, the proposal must be prepared in two parts: (A) Technical Proposal and (B) Business Proposal. For each proposal, the Offeror shall submit all components of the proposal electronically via email, as directed below:
