Choices for Continued Care: An Educational Guide For Patients and Families
This educational guide, developed collectively by the Northwest Denver Connected For Health community and CFMC, provides information about post-acute care settings that are available after being discharged from the hospital. It can help you decide what type of care is right for you or your loved one.
Medications and Supplements Record
This tool will help you keep track of all medications and supplements that you are taking. Keeping an up-to-date list is a great way to inform your doctor informed of all the medications you are taking, including non-prescription drugs, vitamins, and dietary supplements. This can help decrease any possible drug interactions and negative side effects. On the list, be sure to include both the generic and brand names of the drugs, as well as the dosage and times you take the medication. If you have other important things that a doctor or hospital would need to know, including allergies or sensitivities, add it to the list as well.
Personal Health Record
Navigating the health care system can sometimes be confusing. This tool, adapted from Eric Colemanâ€™s Care Transitions Program Personal Health Record and developed collectively by the Northwest Denver Connected For Health community and CFMC, will help you organize all the information you need to feel confident and make the process easier. A personal health record (PHR) is a collection of information about your health, including contact information for your doctors, scheduled appointments, the current medications you are taking, health conditions, allergies and plans for follow-up care.
