The Colorado Foundation for Medical Care (CFMC) is the authorizing agent contracted by the Colorado Department of Health Care Policy and Financing (HCPF) to review Prior Authorization Requests (PARs) for medical necessity.

CFMC provides this review for the following services:

• Diagnostic Imaging - limited to non-emergent Computed Tomography (CT) Scans, Magnetic Resonance Imaging (MRIs), and all Positron Emission Tomography (PET) Scans
• Durable Medical Equipment (DME) - limited to orthotics, prosthetics, power wheelchairs, power scooters, and miscellaneous DME
• Home Health - limited to EPSDT Extraordinary Home Health
• Medical/Surgical - as outlined in the monthly provider bulletins
• Out-of-state, non-emergent admissions and surgical services
• Physical Therapy (PT) and Occupational Therapy (OT)
• Transportation - limited to non-emergent air ambulances, bariatric ground ambulances, commercial flights/trains, meals, and lodging

This secure site allows providers or authorized third party agents to submit PARs to CFMC for the above services, and to view the status and outcome of PARs submitted via this web portal.
