About the Project

CFMC and the American Heart Association have formed a Collaborative to offer $5,000 per hospital and module to hospitals that currently use,  or will adopt, Get With The GuidelinesSM Coronary Artery Disease (GWTG CAD) and/or GWTG HF tools. This opportunity is funded by a grant through the Cancer, Cardiovascular Diseases and Pulmonary Disease Program; Colorado Department of Public Health and Environment.

  • Increase compliance with evidence based guidelines
  • Increase number of hospitals implementing the GWTG CAD & HF modules

Get With The Guidelines is a rapid-cycle, continuous quality improvement initiative for hospitalized patients who have had a coronary event. The program is estimated to be able to save more than 40,000 people annually if the American Heart Association guidelines are implemented at least 90% of the time. The program focuses on building teamwork, identifying a clinical "champion" to drive the program, implementing discharge protocols that include medications and lifestyle modifications, and data collection.
