Description	This session will highlight the opportunity to improve patient outcomes through actions directed at making the patient and/or caregivers more effective and independent in performing health care related activities. Using the prevalent diagnosis of heart failure as an example, general and specific strategies and protocols will be presented that are designed to improve/expedite recovery from illness or surgery, decrease the risk of complications or disease exacerbation, and decrease risk of hospitalization or emergent care use through early symptom identification and intervention.
