Hospital Project Calendar

CFMC is pleased to announce the arrival of our Hospital Project Calendar. In an effort to keep everyone informed of conference calls, WebExs, and meetings we have created a color coded calendar.

The Hospital Projects Calendar currently contains information related to CMSs 8th SOW, Critical Access Hospital conference calls, Colorado's Get With The Guidelines project, American Heart Association National conference calls, Colorado 5 Million Lives Campaign, and IHI-sponsored 5 Million Lives Campaign conference calls.

This calendar will be included in each week's newsletter with a revised date so you will know when it has been updated. Regional Meetings

Save the Date! Heart Failure Facility Lead Workshop Scheduled for
July 25, 2008.

Presented by the Colorado Foundation for Medical Care and the American Heart Association, July 25, 2008 at St. Mary’s Hospital and Medical Center, Grand Junction, CO.

The Facility Lead Workshop will be a one-day educational event that will be offered quarterly in several locations throughout Colorado. The Workshop is designed to help educate and inform facility leads and other team members with necessary processes to start an effective heart failure initiative in their hospitals. Topics include:

  • Building the Heart Failure Team
  • Defining the Heart Failure Quality Measures
  • Locating Heart Failure Patients
  • How to Achieve Medical Staff Buy-In
  • Teaching Staff to Provide Appropriate Discharge Instructions to Heart Failure Patients
  • Making Your Data Help You
  • How to Implement the Tool Kit in Your Facility
