Rural Organization Safety Culture
(ROSC)
An initiative in which hospital teams will work with senior
leadership to assess their base-line organizational safety culture,
determine opportunities for improvement, and implement process
improvements.
Project Objectives:
- Complete and submit the organizational safety culture climate
assessment survey and a patient checklist to report baseline and
remeasurement data.
o Baseline Survey Due Date: 1-12-06
o Remeasurement Survey Due Date: 10-16-07
- Attend Kickoff and quarterly web-ex conference calls and share
best practices.
- Work with CFMC to identify areas of focus to improve the patient
safety climate and implement interventions and systems changes based
on your assessment data results.
Tips on successful completion of survey:
- The survey was designed to be administered to all types of
hospital staff, including clinical and non-clinical. Having varied
departments and staff involved will result in a more complete view
of the safety culture in the hospital.
- Your sample size should be at least TWICE the number of
responses you want to receive. Expect a 50% response rate. (Ex. 400
surveys-200 completed responses)
- Anonymous reporting is important for adequate feedback and
participation. Have a designated hospital point-of-contact for the
survey.
- The survey will take 10-15 minutes-to be completed DURING WORK
TIME. You may consider to administer in team/dept/shift meetings
and/or trainings.
- CFMC will collect the surveys and complete the analysis and
report preparation and data results.

- Supervisor/Manager expectations& actions promoting patient
safety.
- Organizational learning-continuous improvement.

- Feedback & communication about error.
- Non-punitive response to error.
- Staffing.
Hospital-wide Safety areas covered in survey:
- Hospital management support for patient safety.
- Teamwork across hospital units.
- Hospital handoffs & transitions.
