Who We Are
The Colorado Foundation for Medical Care (CFMC) serves as the National Coordinating Center for the Integrating Care for Populations & Communities (ICPC) Aim in the 10th Statement of Work (SOW). National Coordinating Centers (NCCs) support CMS and its network of 53 QIOs in achieving the QIO Program's goals for better care, better health for populations and communities, and lower costs of care through quality improvement. Formerly known as QIO Support Centers, NCCs manage QIO learning and action networks specific to current QIO Program priorities, provide technical assistance and improvement tools, and facilitate communication between the program's national and local levels.
As the ICPC NCC, CFMC is providing QIOs with resources and guidance to build multi-stakeholder community coalitions that reduce avoidable hospital readmissions. Click here for more information about the ICPC Aim.
