Improving Nursing Home Culture (INHC) -
Person Directed Care (PDC) Pilot

The purpose of the Improving Nursing Home Culture (INHC) - Person Directed Care (PDC) pilot was to support a limited number of nursing home communities as they moved forward in their culture change journey.

The goals of the pilot were two-fold:

1. To provide learning opportunities and resources to the nursing home communities that volunteered for the pilot
2. To test materials and interventions to assist the nursing home communities with their individual culture change journeys
