Diabetes

According to the Colorado Department of Public Health and Environment, almost 220,867 persons are diagnosed with diabetes in Colorado and another 92,764 are likely to have the disease but do not know it. Behavioral Risk Factor Surveillance System (BRFSS) estimates indicate the prevalence of diagnosed diabetes in Colorado adults for 2005 was about 4.8 percent.

Over 2,000 Coloradans die every year from diabetes-related causes, and there are over 40,000 hospital discharges for diabetes complications in the state every year.

Working with the Centers for Medicare & Medicaid Services (CMS), CFMC is a committed partner with statewide stakeholders in the fight against diabetes by promoting diabetes education to physicians and beneficiaries. CFMC provides free educational materials to the public and assists physician offices with the delivery of preventive care services to Medicare beneficiaries with diabetes.
