Physician Office Quality Measures

Quality measures for physician offices are divided into three categories: adult immunization, breast cancer screening, and diabetes.

Adult Immunization

Influenza vaccine received in the most recent flu season
Pneumococcal vaccine received ever

Breast Cancer Screening

The proportion of women ages 50-67 at the beginning of a two-year time period who have had at least one mammogram within the time period *
The proportion of women ages 65 and older at the beginning of a two-year time period who have had at least one mammogram within the time period

Diabetes

A1C tested annually
Eye examination performed biennially
Lipid profile performed biennially

* Also a Health Plan Employer Data and Information Set (HEDIS) measure
