Physician Office Quality Measures

Quality measures for physician offices are divided into three categories: adult immunization, breast cancer screening, and diabetes.

Adult Immunization

  • Influenza vaccine received in the most recent flu season
  • Pneumococcal vaccine received ever

Breast Cancer Screening

  • The proportion of women ages 50-67 at the beginning of a two-year time period who have had at least one mammogram within the time period *
  • The proportion of women ages 65 and older at the beginning of a two-year time period who have had at least one mammogram within the time period

Diabetes

  • A1C tested annually
  • Eye examination performed biennially
  • Lipid profile performed biennially

* Also a Health Plan Employer Data and Information Set (HEDIS) measure
