Colorado Rural Health Center
The Colorado Rural Health Center is an independent, non-profit, membership-based organization that serves as the State Office of Rural Health for Colorado. It is a diverse mix of people, programs and places interested in Colorado's rural healthcare that has a growing list of activities and accomplishments. It offers programs and services to ensure that rural communities have access to adequate healthcare.
COPIC Insurance
The Prevention team has formed a partnership with COPIC Insurance. The goal of the collaboration is to provide incentive for practices participating in the education arm of the project. Participants in the CFMC Prevention Project who meet the minimum performance improvement thresholds each quarter are eligible for 1 ERS point each calendar year of the three-year program.
COPIC Insurance Company's mission is to provide a stable market for professional liability and other insurance products, principally in Colorado. This is accomplished through sound management, financial strength, optimum service, risk management, educational activities, strong leadership, and advocacy.
Colorado Academy of Family Physicians
The Colorado Academy of Family Physicians members work to make Colorado the healthiest place to live and the most successful state to practice family medicine. Its mission of the Colorado Academy of Family Physicians is to help Family Physicians thrive so that together we can create a healthier Colorado. The Colorado Academy supports the Prevention project.
Colorado Influenza and Pneumococcal Alert Coalition
The mission of the Colorado and Influenza and Pneumococcal Alert Coalition (CIPAC) is to decrease vaccine-preventable respiratory disease in Colorado through collaborative efforts in education and immunization. The Prevention team stays informed on CIPAC activities and is working to maximize CIPAC's efforts within the physician community.

Health Team Works
Colorado Foundation for Medical Care (CFMC) Prevention project team is collaborating with Health Team Works (formerly the Colorado Clinical Guidelines Collaborative) in connection with their Patient-Centered Medical Home, IPIP projects and Rapid Improvement Activities. The goal of the collaboration is to increase efficiencies for practices interested in participating in multiple projects simultaneously.
Collaboration will include but is not limited to jointly presenting Rapid Improvement Activities connected to prevention, integrating CFMC's Prevention project within the provider-to-provider communications for the Medical Home Project and coordinating improving efforts for IPIP. If your practice is involved in an activity for either organization, which you think may be redundant, please let us know so we can identify additional ways to create efficiencies for you.
Health Team Works is a non-profit coalition of health plans, physicians, hospitals, employers, and other entities working together to implement systems and processes using evidence-based guidelines. The group works at a practice level to help improve processes in care, including those that may involve disparate groups.
