Medicare Providers
The Right Care for Every Person, Every Time
As the Medicare Quality Improvement Organization (QIO) for Colorado, CFMC's overarching mission is improving health and healthcare for all Medicare beneficiaries. Currently, we are collaborating with healthcare providers, patients and partners in Colorado to achieve the bold QIO Program goals established by CMS which are, better patient care, better health for people and communities, and lower costs of care through improvement.
To achieve our goals, we are engaging a broad spectrum of providers, patients and quality stakeholders. We will be establishing Learning and Action Networks, an improvement initiative that brings together healthcare professionals, patients and other stakeholders around an evidence-based agenda to achieve rapid, wide-scale improvement. We will also offer providers direct technical assistance as requested and needed. This includes consultation, knowledge management, face-to-face/hands-on teaching, data analysis and assistance creating a sustainable infrastructure.
Specific areas of work include:
- Beneficiary and Family-Centered Care
- Integrating Care For Populations and Communities
- Improving Individual Patient Care
- Improving Health for Populations and Communities
- Integrating Care For Populations and Communities National Coordinating Center
- Learning and Action Networks (LANs)
